University of Minnesota Medical School

Department of Neurology

Postgraduate Fellowship

History Project

Dear Fellow Neurologist,

Dr. William Kennedy has taken on the monumental task of compiling a history of the Neurology Department at the University of Minnesota.  A major component of this historic account will be us, the residents, who are the products of the postgraduate educational program associated with the Neurology Department.

The success of the entire project relies on the degree of cooperation of all of us who have completed a residency at the University.  We ask your indulgence to supply the answers to the questions posed below.  Please do not feel confined to the questionnaire format. Pass on your experiences, during residency and present, serious, humorous even irreverent! Tell us about your career as a neurologist. Your fellow trainees will be happy to learn about you. Since the history will be on a web site the length of your response is not a problem.

Thank you for sharing your memories with us.

                                                                                             Terrance D. Capistrant

                                                                                             James R. Allen

                                                                                             William R. Kennedy

When responding to the attached questionnaire:

Using email send the below word document to kenne001@umn.edu with CC. to capis004@umn.edu

Please note:  Your responses will be made available for review by your peers by accessing the history link of Bill Kennedy’s web site at: kennedylab.med.umn.edu. Eventually the history will be on the Department of Neurology web site http://www.neurology.umn.edu/
